A5 Football Camp Waiver, Release of Liability, and Assumption of Risk Agreement

Acknowledgment and Assumption of Risk I, the undersigned Participant (or Parent/Guardian on behalf of the minor Participant), acknowledge that participation in A5 Football Camp activities, including but not limited to drills, training, scrimmages, and related physical exercises, involves inherent risks of physical injury, including sprains, strains, fractures, concussions, heat-related illnesses, and other serious or permanent injuries, up to and including death. These risks may arise from the nature of football, equipment use, field conditions, actions of other participants, or unforeseen circumstances.

I voluntarily assume all such risks, known and unknown, and agree that A5, LLC, its directors, officers, employees, coaches (including Siaosi Aiono), volunteers, agents, and affiliates (collectively, "Released Parties") shall not be held responsible for any injury, illness, damage, loss, or expense resulting from participation in the Camp.

Release of Liability In consideration of being permitted to participate in the A5 Football Camp, I hereby release, waive, discharge, and covenant not to sue the Released Parties from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by the Participant, whether caused by negligence of the Released Parties or otherwise, while participating in the Camp or while on the premises where the Camp is conducted.

This release extends to any claims for personal injury, property damage, or wrongful death, and includes any claims brought by me, the Participant, or on behalf of the Participant by heirs, assigns, or legal representatives.

Medical Authorization and Insurance I certify that the Participant is physically fit to participate in the Camp and has no medical conditions that would prevent safe participation, except as noted here:

I authorize the Released Parties to administer or arrange for emergency medical treatment if necessary, and I agree to bear all costs associated with such treatment. I understand that A5 does not provide medical insurance and that I am responsible for any medical expenses incurred.

Indemnification I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur due to the Participant's presence or participation in the Camp, whether caused by the Participant's negligence or otherwise.

Photo/Video Release I grant A5 permission to use photographs, videos, or other recordings of the Participant taken during the Camp for promotional, educational, or archival purposes, without compensation.

Governing Law This Agreement shall be governed by the laws of the State of California. If any provision is found invalid, the remaining provisions shall remain in full effect.
